This creates an obvious question: If the care homes are paying the agency, does the agency act in their interest rather than your family’s interest?

The honest answer is that this depends entirely on the integrity of the placement agency. Step 1: Initial Consultation

Your process begins with a phone call or in-person meeting with an Elder Answers placement advisor. This conversation has three purposes:

  • We learn about your loved one’s current situation — what has changed, what the timeline is, what care needs have emerged
  • We assess care level requirements: ADL needs, cognitive status, behavioral considerations, medical complexity
  • We understand your family’s priorities: location, budget, room type, cultural or dietary preferences, specific concerns

This conversation typically takes 30–45 minutes. If the situation is urgent — a hospital discharge pending, a recent fall, a safety crisis at home — we can conduct a rapid assessment and begin identifying options immediately.

Step 2: Matching to Appropriate Homes

Based on the assessment, we identify board and care homes and/or assisted living communities in San Diego that are a strong match on care level, personality fit, location, and budget. We only recommend homes we have personally vetted.

Our vetting process includes visiting every home in our network, reviewing CDSS inspection and citation history, speaking with operators and observing staff, and monitoring family feedback after placements. We do not send families to homes we have not personally evaluated.

We typically present 3–5 options at this stage, with a brief summary of why each is worth considering for your specific situation.

Step 3: Guided Tours

We accompany you on every tour. This is one of the most valuable services we provide.

Touring a board and care home without experience is difficult. You may not know what to look for, what questions to ask, or how to interpret what you’re seeing. Our advisors have been on hundreds of tours. They know what good care looks like — and what warning signs to watch for. They ask the questions families forget to ask. They observe the staff-resident interactions that reveal more than any marketing brochure.

We also help manage the emotional weight of the touring process. For many families, visiting care homes is a deeply difficult experience. Having an experienced advisor by your side provides both practical guidance and emotional support.

Step 4: Decision Support

After touring, we debrief with you to help process what you saw. We share our professional assessment of each home you visited. We answer follow-up questions. If one home stood out as a clear choice, we help facilitate next steps. If you’re undecided, we may identify additional homes to visit or gather more information.

We do not pressure families to make decisions on a particular timeline. Our goal is to find the right fit, not to close placements quickly.

Step 5: Contract Review and Move-In Coordination

Once a home is selected, we help you review the admission agreement before signing — making sure you understand the fee structure, care level provisions, rate increase policies, and termination terms.

We then coordinate with the care home on the specific move-in date and logistics. For complex situations involving transitions from a hospital or skilled nursing facility, we work with the discharge planning team to ensure a smooth handoff.

Step 6: Post-Placement Follow-Up

Our service doesn’t end at move-in. We check in with families in the weeks following placement to make sure the transition is going well. If there are concerns — about care quality, communication with staff, adjustment challenges — we help address them. In the rare cases where a placement is not working out, we help find an alternative.

California does not currently license senior placement agencies as a category.
